New to Medicare
If you are newly eligible for Medicare Part A (Hospital Insurance) and Part B (Medical Insurance), you may apply online. Because you may have to pay a premium for Part B coverage, you can turn it down. You do not have to claim Social Security retirement benefits apply for Medicare. If you don’t currently receive any Social Security income, you may get a bill for your Part B Medicare.

If you’re eligible at age 65, your initial enrollment period begins three months before your 65th birthday, includes the month you turn age 65, and ends three months after that birthday.
If you choose not to enroll in Medicare Part B and then decide to do so later, your coverage could be delayed and you may have to pay a higher monthly premium for as long as you have Part B.

Already have Medicare Part A
If you already have Medicare Part A (Hospital Insurance) and you’re eligible to enroll in Medicare Part B (Medical Insurance) through a Special Enrollment Period (SEP), you have options for how to apply. You can apply online or you can mail your completed CMS 40B, Application for Enrollment in Medicare – Part B (Medical Insurance) to your local Social Security office.
​You’ll also need to send CMS L564 – Request for Employment Information, and a required proof of employment, Group Health Plan (GHP), or Large Group Health Plan (LGHP) coverage with your 40B form.
You can also fax your enrollment forms and evidence of employment and health coverage to your local Social Security office.

Important Dates
INITIAL ENROLLMENT PERIOD (IEP):
65th birthday month + 3 months before
AND 3 months after = 7 months
Your Initial Enrollment Period (IEP) is your first chance to enroll in Medicare.
Special Enrollment Periods (SEPs):
Certain circumstances apply
Effective Date: Date of coverage varies
SEPs allow you to delay your enrollment. Circumstances include leaving
an employer health plan or moving your primary residence. Visit Medicare.gov for a complete list of qualifying circumstances.
Medicare Open Enrollment Period (OEP):
January 1 to March 31
Effective Date: First of the month after the plan gets your request.
If you miss your IEP or SEP, you get another chance to enroll during OEP.
Annual Enrollment Period (AEP):
October 15 to December 7th
Effective Date: January 1
During AEP, you may add, drop or switch your Medicare coverage.
